Rather poorly exposed family portrait

When I first saw this photo I assumed that the photographer had intended to take a flash lit portrait, but the flash didn’t fire. When I looked in more detail however, I think the flash did fire, because I can see bright reflections in the glasses the middle woman is wearing, and from the cheeks of the man, so I think the exposure issues may be to do with setting the wrong aperture or shutter speed.
